I. AVAILMENT OF ASSISTANCE IN CRISIS SITUATIONS |
1. Get your Number Card from the Social Welfare Clerk and register in the Client’s Logbook. Wait for your number to be called. |

2. When your number is called, proceed to the Social Worker on duty for interview and assessment. |

3. The Case Study Report together with other supporting documents will then be prepared by the attending Social Worker for the approval of the Mayor. |

II. ISSUANCE OF ID CARDS AND PURCHASE BOOKLETS TO SENIOR CITIZENS |
1. Go to the Office of the Senior Citizens’ Affair to fill-up Application Form for Membership. |

1. Go to Social Worker on duty and register your names in the Marriage Counseling Logbook. |
